School Bus Driver Physicals
DOT Drug and Alcohol Testing: A basic DOT exam does not include drug and alcohol testing, but it can be performed at the same time as your regular exam if required by your company. Drug and alcohol testing requires a separate test and is not included in the cost of a basic DOT exam.

What to Expect from Your Exam
Step 1: Medical History
To provide a better understanding of your overall health, you will be asked to complete a brief medical history questionnaire that goes over your past injuries, surgeries, medications and current and past health issues. When you schedule your appointment we will email or text a link to complete your paperwork online before your visit. Filling out this form before your visit will help us get to your exam more quickly.
Step 2: The Exam
When you arrive at our office you will be ready to start your comprehensive physical exam which includes: urinalysis, height and weight, vision and hearing, eyes and ears, heart and lungs, range of motion, reflexes, blood pressure and pulse, and abdominal exam.
Step 3: Paperwork
Our staff will complete all of the necessary paperwork and upload your certificate to the Federal and NC State agencies on your behalf. After all of the paperwork is complete, you will leave with a shiny new copy of your very own DOT Medical Card. We also keep all of your records on file, so if you need a new copy, just let us know.
Preparing for Your Exam
A DOT Physical at Family Circle Chiropractic only takes about 30 minutes. We accept cash, checks, and all major credit cards. Before your exam, compile your medical records, put together a list of medical conditions you have been diagnosed with, prepare a list of medications and the name of the prescribing doctor, and take note of any medical devices that you require.
On the day of your exam use this checklist of things you should bring (where applicable):
Your Driver’s License
Pertinent Medical Records
A Complete List of Your Medications, including strength and dosage, for both prescription and over the counter drugs
Eye glasses
Hearing aids
Copy of your most recent stress test (for heart conditions)
Copy of your latest sleep test/CPAP use report (for sleep apnea)
Letter from your prescribing doctor stating your medications are well-monitored and you don’t have side effects (for medications for blood pressure, cholesterol, or diabetes)
Right before your exam: Make sure you drink a glass of water and come in with a full bladder, as you will have to provide a urine sample.
