Feel free to park anywhere in the Print Haus Plaza. Come in the doors near the road sign and turn right inside the vestibule to find our office. Our office is fully handicapped accessible. For a map to our office, click here.
What should you bring:
Please bring your ID/driver's license with you.
For Patients with Medicare or ages 65+:
Please bring your traditional, red, white and blue Medicare insurance card, NOT YOUR MEDICARE REPLACEMENT CARD. We are required by federal law to have a copy of this card on file BEFORE we can treat you. If you bring us a Humana, United Healthcare, Aetna, Blue Cross etc. card instead of your red white and blue card, we have to send you home to get the correct card instead of treating you. If you do not have a traditional red, white and blue Medicare card, PLEASE Call Medicare to get one. There are no exceptions! It breaks our hearts to turn away people who need us, but we have to comply with federal law and we need the correct card on file. More about fees and how we bill insurance here.
Please bring medication lists, previous x-rays or other medical documents you would like us to have on file.
Please do not bring your pets. As much as we would love to have your furry friends and family in our office, some of our patients are allergic and we need to be respectful of them. Service animals are an exception and are of course welcome in our office.
New patient paperwork:
The easiest way to complete your paperwork is by scheduling online above. Follow the links to the new patient paperwork and simply type your information into the form and click submit, and it will automatically upload to our system. While the online paperwork is easier and faster, we do have a paper version available in the office if you would prefer. We tried to make it as short and painless as possible. If you choose to complete our paperwork in the office, we ask that you arrive 15-20 minutes early to allow time to complete it. This helps us stay on schedule so we can see you as promptly as possible.
The History and Exam
To begin, our doctors will sit down with you and discuss your concerns and health goals before beginning the examination. All new patients are required to have an exam which will be tailored to your specific issues and usually includes vital signs, range of motion and an orthopedic and neurologic workup. This allows us to determine exactly what is causing your problem so we can recommend the most appropriate treatment plan to get you better faster. We will explain our exam findings with you in detail and go over any questions you might have and what you are looking to get out of your care. We will then work with you to determine a treatment plan that meets your goals. If your problem is not best treated by us, we will refer you to the most appropriate provider to make sure you get the best care for your specific issue. We will never treat you if we believe our treatment will not be effective or over-treat you just to keep you coming in to us. We believe this is highly unethical and unprofessional. Our goal is to get you better as soon as possible so you can live a happy, pain free life.
Additional testing
It is possible that additional testing may need to be done in order for us to reach the correct diagnosis. This testing may include blood work, urinalysis, nerve or muscle conduction testing (EMG/NCV), or diagnostic imaging such as X-rays, CT, MRI, or ultrasound. If any of these additional forms of testing are needed, we will refer you to the appropriate lab facility.
Why we don't do X-Rays
There are several reasons why we prefer to refer out to an imaging center for x-rays. Most of the time they are not needed, and overusing them can cause harm to the patient. While chiropractors are fully trained to take and read x-rays as well as most other forms of advanced imaging, imaging centers have better equipment, more skilled radiology technicians, more skilled radiologists, and are usually cheaper for the patient. Simply put, they do it better and take better images which give us more useful information. We refer out to make sure you get the best care possible.

Treatment
